Abstract

THE EFFECT OF LOW-POWER LASER IN COMBINATION WITH PHOTOSENSITIZER AND ANTIMICROBIAL AGENTS ON STREPTOCOCCUS VIRIDANS CAUSING DENTAL CARIES IN IRAQI PATIENTS

ABSTRACT

Background: The structure of tooth is enamel, dentine, cementum, pulp, nerves, and periodontium .Dental caries is an infectious, communicable, multifactorial disease in which bacteria dissolve the enamel surface of a tooth.Streptococcus viridans is one of these infectious agents leading to dental caries. Methodology: This study was conducted in Laboratory of Microbiology, Department of Microbiology, College of Medicine, Tikrit University during the period between December 2006 to July 2007. One hundred patients were included in this study attending the dental department in Tikrit Teaching Hospital. Exposure of the 4 strains of Viridans Streptococci to low-power diode laser light; diode laser in combination with photochemical agent (povidone-iodine); then all these variable combinations were combined with different antibiotics (vancomycin ,doxycyclline ,amoxycillin, cefotaxime, erythromycin, metronidazole, augmentin and chlorhexidine had a significant effect in killing the bacteria.Minimal inhibitory concentrations and time needed were estimated. Results: It was found that povidone-iodine was a good photosensitizer with exposure times 32 and 64 minutes to sensitize Viridans Streptococci for killing by low-power diode laser light, and there were synergistic effects between laser light and photosensitizer. From this study , it was concluded, that Streptococcus viridans were killed by low-power diode laser in combination with photochemical agents, especially laser-povidone-iodine-antibiotics- and chlorhexidine combination which was considered the most effective and most suitable than other combination because of less concentration of antibiotics and chlorhexidine and minimum time which required for killing this bacteria. Conclusions: Exposure of Viridans streptococci isolates to low-power diode laser light alone had no significant effect in killing bacteria. While exposure of them to low-power diode laser light in the presence of photosensitizer (povidone-iodine)with antibiotics and chlorhexidine led to sharp reduction in total number bacteria which was statistically significant. This combination considered the most effective and most suitable than other combinations because less concentration of antibiotics and chlorhexidine, and minimum time required for killing this bacteria by laser light.
