FROM CORRESPONDENCE TO SCIENTIFIC VALIDATION: AN EVIDENCE-GRADED CRITICAL REAPPRAISAL OF THE AYURVEDIC AGNI–AMA FRAMEWORK IN THE CONTEXT OF GUT MICROBIOME SCIENCE

Abstract Background: Ayurveda, the ancient Indian system of medicine, conceptualizes digestive health through two cardinal constructs: Agni (the biological transformative fire) and Ama (the pathological residue of impaired digestion). Contemporary gut microbiome science has identified functionally analogous phenomena — eubiosis/dysbiosis, intestinal permeability, endotoxemia, and microbial metabolite signaling — that bear striking parallels to these classical constructs. Objective: This integrative review aims to systematically explore conceptual, mechanistic, and translational correspondences between the Ayurvedic Agni-Ama framework and modern gut microbiome science, and to evaluate available evidence for Agni-modulating interventions through a microbiome lens. Methods: A comprehensive narrative review was conducted using PubMed, SCOPUS, AYUSH Research Portal, and DHARA databases. Classical Ayurvedic texts (Charaka Samhita, Sushruta Samhita, Ashtanga Hridayam) were reviewed alongside peer-reviewed microbiome literature published between 2000 and 2024. Results: Agni demonstrates operational equivalence to the ensemble of enzymatic, immunological, and microbial processes governing gastrointestinal homeostasis. Sama Agni (balanced digestive fire) maps onto the concept of eubiosis, while its four dysfunctional states (Vishama, Tikshna, Manda, and Sama with Ama) correspond to distinct dysbiosis phenotypes. Ama parallels microbially derived toxic metabolites including lipopolysaccharides (LPS), trimethylamine N-oxide (TMAO), secondary bile acids, and other circulating endotoxins. Classical Ayurvedic interventions — including Triphala, Trikatu, and Panchakarma — demonstrate mechanistic plausibility through documented microbiome-modulating effects. Conclusion: The Agni-Ama paradigm offers a sophisticated pre-modern conceptual architecture that aligns substantively with contemporary understanding of gut microbiome-host interactions. Rigorous clinical and experimental research at this interface holds significant promise for integrative medicine, personalized nutrition, and novel therapeutic discovery.
