PHARMACODYNAMIC ACTION OF VIRECHANA KARMA IN MADHUMEHA (TYPE 2 DIABETES MELLITUS

Abstract Background: Madhumeha, described under the spectrum of Prameha in classical Ayurvedic literature, closely resembles Type 2 Diabetes Mellitus (T2DM), a chronic metabolic disorder characterized by insulin resistance, β-cell dysfunction, chronic low-grade inflammation, oxidative stress, and metabolic endotoxemia.[1,3] Ayurveda attributes its pathogenesis to Kapha-Pitta aggravation, Medo-dhatu vriddhi, Agnimandya, and Srotorodha.[4,7] Objective: To explore the pharmacodynamic basis of Virechana Karma in Madhumeha through classical Ayurvedic principles and contemporary molecular mechanisms. Materials and Methods: Classical Ayurvedic texts including Charaka Samhita, Sushruta Samhita, and Ashtanga Hridaya were critically reviewed. Contemporary biomedical literature addressing insulin resistance, inflammatory signaling, oxidative stress, gut microbiota, bile inflammatory cytokines (TNF-α, IL-6, IL-1β), improve insulin signaling (IRS-1/PI3K/Akt pathway), reduce oxidative stress, alter gut microbial composition, enhance bile acid receptor activation (FXR/TGR5), and recalibrate neuroendocrine stress responses. These interconnected mechanisms correspond to classical concepts of Agni Deepana, Ama Pachana, Srotoshodhana, and Dosha Shamana. Conclusion: Virechana may exert multi-system pharmacodynamic effects relevant to Madhumeha. Rigorous translational research integrating molecular biomarkers and randomized clinical trials is essential for scientific validation and global integration.
