HISTOPATHOLOGICAL EVALUATION OF PAPILLARY UROTHELIAL LESIONS OF THE URINARY BLADDER IN A SAMPLE OF IRAQI PATIENTS

. Abstract Background: The most common neoplasms of the urinary bladder are papillary tumors that vary histologically and cytologically from very well differentiated to highly anaplastic patterns. The purpose of the study was to determine the histopathologic characteristics of papillary urothelial lesions of the urinary bladder and their correlation to age, sex, clinical presentation, size, grade, pathological stage, single or multiple, primary or recurrent. Methods: This is a retrospective study carried out on 100 cases reported from January 2019 to September 2021 in department of pathology, Baghdad Medical City /Baghdad /Iraq. All statistical analyses were performed using SPSS. Variables included: age group, gender, site, size, grade and pathological stage. Results: The age ranged from 22-85 years with a mean age of presentation was 62.55± 14.8 years, the most frequent age group was 61-70 years with 36 (36%) patients followed by those who are 71 years or older with 32 (32%) patients. Males predominated with 91 (91%) males 9 (9%) females, with 10.1:1 male to female ratio. The mean size of lesion was 2.4± 2.18 cm ranging from 0.1cm to 12cm. The most common location was lateral wall of bladder with 57(57%) lesions. There was one case of papillary urothelial neoplasm of low malignant potential (1%) and 99 (99%) papillary urothelial carcinoma, 49(49%) of them were low grade while High grade tumors account for 50 (50.0%) cases. Low grade papillary carcinoma was observed most frequently in 61-70 years age group with 21(42.9%) patients, followed by ≥71 years with 12(24.5%) patients, while high grade was most frequently seen in ≥ 70 years patients with 20 (40%) patients followed by those who are 61-70 years with 15 (30%) cases.
