. Abstract Portal hypertension, characterized by increased pressure within the portal venous system, is a significant complication often associated with cirrhosis. The key diagnostic parameter is the portal pressure gradient, with a gradient of 6 mmHg or more indicating the presence of portal hypertension and a gradient exceeding 10 mmHg considered clinically significant. This condition arises from increased resistance to portal blood flow, occurring either within the liver (cirrhosis) or externally, such as in portal vein thrombosis or Budd-Chiari syndrome.
