MATERNAL AGE, BODY MASS INDEX AND SUCCESS RATE OF VAGINAL BIRTH AFTER CESAREAN SECTION IN AL HADA, ARMED FORCES HOSPITAL, 2015- 2016

Abstract Rates of vaginal birth after caesarean (VBAC) is rising all over the world. Attempting a VBAC is a safe and appropriate choice that must be offered to most women who have had a prior caesarean delivery. Aim: To identify predictive factors which is maternal age and body mass index associated to vaginal delivery success in patients with a prior cesarean delivery. Method and Sample: Retrospective case control analyzing data from the delivery unit of Alhada Armed forces hospital which includes data will be collected from 2015 to 2017. The study sample consists of (134) pregnant woman whom want to deliver CS or VBCS. Aged between 20-45 years old and with BMI of 18 – 40. Results: the majority of the pregnant women were less than 39 years old (92.8%). Regarding to body mass index, 16.4% were normal, 38.1% were overweight and 45.5% were obese. Moreover, about two third of the women had been pregnant one to four times (71.6%), and more than half of them had given one or two times birth to a fetus with a gestational age of 24 weeks or more (58.2%). Pregnant women had previous caesarean section for several reasons (56% fetal distress, 25% mal presentation, 5% multiple gestation, 4% placenta previa, 10% other). In addition, two third of pregnant women had not have any previous miscarriage (720.1%). Of our sample (134) 64.9% delivered via SVD and 32.1% delivered by CS. VBCS sample delivered successfully with only two deliveries have newborn with complication such as uterine rupture and uterine inversion. The presented results showed success of VBAC in 17.6% of normal weight, 38.5% of overweight and 44% of obese. Our results showed 36.3% of our sample at the age between 20-29, 58.2% between 30-29 while only 5.5% over 40 and all of them delivered successfully by VBCS, except one case at the age 34 and normal BMI have post-partum hemorrhage. No significant differences regard to age and BMI of pregnant women (p-value >0.05). Conclusion: Vaginal delivery after cesarean doesn’t cause morbidity or mortality either to mother or the neonate.
